It likely would have to make the entire game work the same throughout the in-game history, though, so if you'd model it like Ancient world stuff, you'd likely not be able to switch over to a Medieval political model.

It's not a perfect comparison, of course, which is why Paradox themselves won't ever go for it, but it may well appeal to some people wanting to create a mod that expands upon things or changes the world. In that regard, they were much like the post-Medieval pre-democracy countries (or more recent constitutional monarchies). On the smaller scale (with dukes and counts and barons), most of the ancient empires and kingdoms either had people appointed or elected for those positions. While the ancient world has some similarities with the medieval world in terms of politics, this is mostly on the larger scale.
